We partner with Medicare and Medicaid patients with chronic medical conditions on their health journey. We are not looking to replace your doctor, but add additional services. Some services we provide:

·  Connecting Patients with Specialists: It can be difficult for patients to find specialists for their health conditions. Many primary care clinics may not have specialist partners. We help patients find the specialists they need. 

 

· Insurance Coordination: Nobody can promise they can get insurance companies to approve a treatment or procedure. However, we can greatly improve your odds. We do this by following up on claims, submitting relevant documentation, and acting as a point of communication between your insurance and your doctor(s). Also, if it isn't possible to get insurance coverage for a medical treatment or medical equipment, we can help you find alternative resources to pay for what you need. ​

· Explaining Lab Results: Your primary care doctor should be, and often is your principal resource explaining your lab results. However, even great primary care providers can get overwhelmed with the demands of their schedule. They may not have the time to fully explain what certain labs mean. We can help with this.
